RESUMEN

A pneumonia outbreak of unknown origin was reported in Wuhan, China in late December. This virus, called coronavirus-2, has an impact on the respiratory tract, leading to acute respiratory syndromes. In 2020, this virus was declared a pandemic by the World Health Organization since it caused a high number of deaths worldwide. In addition, this pandemic has had a negative impact on the world economy, focusing the attention of the practitioners on the resource management in health structures. This work was carried out to evaluate the effects of the pandemic on the ordinary hospitalization activities of the Department of Ophthalmology at "A. Cardarelli” based in Naples (Italy). The dataset was evaluated using statistical analysis techniques and logistic regression. The results, for this department, did not show significant differences when comparing the health variables of the pre-pandemic year (2019) with the pandemic year (2020). © 2023, The Author(s), under exclusive license to Springer Nature Switzerland AG.

RESUMEN

In December 2019, SARS-CoV-2 broke out, which raised great attention worldwide. In fact, it was essential to reorganize the management of economic, infrastructural and medical resources to deal with the inadequate preparation of medical practitioners for this emergency. It was evident that the global health, medical and scientific communities were not adequately prepared for this emergency, so during the pandemic. In this paper, data extracted from hospital discharge records of the Department of Urology of the A.O.R.N "Cardarelli” in Naples, Italy, were used. This work is an extension of a previous work, whose goal concerned how admission procedure in the Urology department of the "San Giovanni di Dio and Ruggi d'Aragona” hospital has been affected by COVID-19 pandemic. In this work we compare the results obtained for the patients of the University Hospital "San Giovanni di Dio and Ruggi d'Aragona” of Salerno and the patients of the A.O.R.N. "Antonio Cardarelli” of Naples (Italy). Data have been extracted from both hospitals discharge records of the Departments of Urology. Experimental analysis performed comparing pre-pandemic data with those collected during the epidemic showed an in-crease in the number of emergency hospitalizations and a decrease in planned pre-admission hospitalizations. © 2023, The Author(s), under exclusive license to Springer Nature Switzerland AG.

RESUMEN

In the last few years, the COVID-19 pandemic has strongly affected different hospital departments, revealing their major weaknesses. For this reason, this emergency has been a driver for healthcare transformation in a very short interval of time in order to optimize the resources, minimize costs and simultaneously increase the caring services, also limiting over-occupancy in wards, especially emergency ones. One of the main factors for assessing the efficiency of a department is associated with how long a patient stays in the facility (LOS). This bicentric study investigated how COVID-19 has modified the activity of the Complex Operative Unit (C.O.U.) of Neurology and Stroke of the University Hospital "San Giovanni di Dio e Ruggi d'Aragona” of Salerno (Italy) and the hospital A.O.R.N. "Antonio Cardarelli” of Naples (Italy). In the work data for the year 2019 (in the absence of Covid-19) and in the year of Covid-19 pandemic 2020 were considered. This work used the logistic regression technique to study the following variables: age, gender, length of stay (LOS), relative weight of DRG and mode of discharge. The analysis shows that in 2020 there was a greater adequacy of admissions, with an increase in the relative weight of DRG. And the statistical analysis obtained the following significant variables: gender, age, relative weight of DRG and discharge mode. © 2023, The Author(s), under exclusive license to Springer Nature Switzerland AG.

RESUMEN

CoViD-19 pandemic caused a severe changing of healthcare facilities activities. Specifically, one of the most affected areas are the Department of Emergency Surgery that have been reorganized to face the emergency giving priority to urgent procedures at cost of those which could be deferred. This study evaluates the impact of the pandemic on the departments of two different Italian Hospitals: "San Giovanni di Dio and Ruggi d'Aragona” University Hospital in Salerno and the AORN "A. Cardarelli” of Napoli. Two different analyses (statistical and machine learning) have been provided for investigating patients in 2019, as an example of the normal activity before the pandemic, and those recorded in 2020, in which the pandemic reached its peak. The evaluation performed showed an increase in the urgent hospitalization and Diagnostic Related Group while transfers to Social Care Residences (RSA) decreased in both the Hospitals, even if the steepness of these changes are consistent with the starting values. © 2023, The Author(s), under exclusive license to Springer Nature Switzerland AG.

RESUMEN

The pandemic related to the Covid-19 virus that began in 2019 in China and then extended to the rest of the world has led to changes in the management of almost all clinical specializations. The main adaptations are due not only to changes in managerial management to better address organizational difficulties but there have also been variations from a treatment and care management point of view with respect to different clinical sectors including that relating to the Neurosurgery sector. In our analysis, the activity of the Department of Neurosurgery in AORN "A. Cardarelli” in Naples (Italy) was analysed. In particular, our analysis aims to investigate variables pre and post pandemic, comparing information gathered in 2019 and 2020. In the specific case, the hospitalizations of 2177 patients were considered in order to understand the influences that the Department has suffered due to the difficulties linked to the pandemic. © 2023, The Author(s), under exclusive license to Springer Nature Switzerland AG.

RESUMEN

Background. The CoViD-19 pandemic has hit the public health system in many nations hard. The long incubation period, the high infectious rate, the varied manifestation and the lack of effective treatment make managing the transmission of the disease difficult. For this reason, the health focus has been shifted to its management, thus limiting the activity carried out by surgical specialties. In neurosurgery, the complexity and importance of treatments in some cases requires timely intervention that cannot be delayed. Methods. In this study, the demographic and activity data of patients discharged from the Complex Operative Units (COU) of Neurosurgery and Neurosurgical Clinic of the "San Giovanni di Dio e Ruggi d'Aragona"University Hospital of Salerno (Italy) in the years 2019 (pre-pandemic) and 2020 (during the pandemic) were analyzed using statistical test and logistic regression. Results. Both statistical tests and logistic regression showed a significant increase in the weight of the Diagnosis Related Group (DRG) obtained from the COUs, demonstrating an improvement in the appropriateness of the services provided. Conclusion. This work demonstrates how a more consistent use of the hospital due to fear and containment measures has improved the performance of the COU. © 2022 ACM.

RESUMEN

The CoViD-19 pandemic is an international public health emergency. The management of the hospitals found themselves facing an ever-increasing demand for beds, especially for intensive care, thus having to make critical decisions in a short time. A shared action was to block elective surgery deemed deferrable and to transfer resources to the management of the pandemic. The activity of the surgical departments has therefore undergone substantial alterations. In this study, logistic regression and statistical analysis was used to characterize this difference for the Urology Department in "San Giovanni di Dio and Ruggi d'Aragona"University Hospital of Salerno (Italy) by analyzing a set of variables and compared the data obtained in the year 2019, pre-pandemic, with that recorded the following year, in the height of the pandemic. The results show an increased number of emergency hospitalizations as well as scheduled hospitalizations with pre-admission. © 2021 ACM.

RESUMEN

CoViD-19 has placed the health systems of many countries in further crisis. The elective surgeries were canceled and the staff of several departments, including general medicine, underwent a reallocation to deal with the health emergency. In a context of economic fragility, in recent years the use of indicators for measuring health quality and performance has acquired more and more importance. Access limited only to emergency-urgency cases within hospitals has however produced a benefit in improving the appropriateness of admissions. In this study the value of parameter sets obtained in year 2019 (pre-pandemic) and year 2020 (during the pandemic), including Length of Stay and Diagnostic Related Group (DRG) Weight, of the Department of General Medicine of the University Hospital of Salerno 'San Giovanni di Dio e Ruggi D'Aragona' in Salerno (Italy) were compared using statistical analysis and logistic regression. The statistical analysis shows an increase in the DRG Weight in 2020, so an increase of the complexity of the cases treated and a greater appropriateness of hospitalizations. © 2021 ACM.

RESUMEN

Since the first case recorded in China in 2019, CoViD-19 has overwhelmed the health systems of all countries. The highly complex request for assistance combined with the fear of contagion have changed the normal activity of hospitals. Conversely, as the pandemic spreads, fewer people are going to the Emergency Department (ED) for non-CoViD diseases. In this study, logistic regression and statistical analysis were used to investigate how the pandemic changed the activity of the Emergency Medicine Department of "San Giovanni di Dio and Ruggi d'Aragona"University Hospital of Salerno (Italy). Patients admitted in 2020 have a higher Length of Stay (LOS) and the mode of discharge is mostly "at home". While the discharge modality 'transferred to another regime in the same hospital' had significantly decreased in order to counter the internal contagion. © 2021 ACM.

RESUMEN

The CoViD-19 pandemic since December 2019 has rapidly spread around the world and to date does not seem to stop its run. The world has paid a very high price in terms of human lives lost and economic repercussions. Health systems have generally been severely tested. The strong demand for CoViD-19 patients has resulted in the blocking of deferred elective procedures and a reconversion of beds and human and technological resources towards the management of the pandemic. In this work, the activity of the Department of Ophthalmology in "San Giovanni di Dio and Ruggi d'Aragona"University Hospital of Salerno (Italy) was analyzed. Specifically, the value obtained from a set of data in the year 2019 (pre-pandemic) was compared with that obtained in the following year, in the height of the pandemic. The results show that patients admitted in 2020 have a shorter length of stay. © 2021 ACM.

RESUMEN

The CoViD-19 pandemic has unfortunately not spared the children. In most cases, the infection is asymptomatic and the serious outcomes are much lower than in adults. Despite this, as the hospital is committed to treating infected patients and limiting the spread of infections, pediatric surgery has also undergone the blocking of procedures deemed deferrable like other surgeries. In light of this, in this study it was decided to analyze how the activity of the Complex Operative Units (COUs) of Pediatrics and Pediatric Surgery in "San Giovanni di Dio and Ruggi d'Aragona"University Hospital of Salerno (Italy) was affected by the pandemic. Both statistical analysis and logistic regression show an increase of patients' average age and of weight of Diagnostic Related Group (DRG). © 2021 ACM.

RESUMEN

Hospital admissions and surgical activities have undergone substantial changes due to the spread of the coronavirus 19 (CoViD-19) pandemic. To date, however, there are in increasingly number studies that have quantified the effect of the pandemic and its protocols on hospital activity. Unlike other studies, which identify protocols for the containment of infections, in this study, logistic regression and statistical analysis were used to evaluate the type of patients admitted to the Complex Operative Unit (COU) of General Surgery and Day Surgery and Breast Unit in "San Giovanni di Dio and Ruggi d'Aragona"University Hospital of Salerno (Italy) in CoViD-19 era. Specifically, the aim is to study how hospitalized patients has changed by investigating the relative parameters extracts from hospital discharge forms, comparing year 2019 and year 2020. The result shows that in year 2020, the weight of Diagnostic Related Group (DRG) was significantly higher and it highlighted an increased use of the emergency hospitalization. © 2021 ACM.

RESUMEN

CoViD-19 caused a significant alteration of the normal activity of hospital facilities. In particular, the surgical departments have been converted and reprogrammed to cope with the emergency, giving priority to urgent procedures that cannot be deferred. To this must be added the actions implemented by governments, such as the lockdown, which forced people to stay in their homes. In this study it was analyzed how the pandemic and the actions taken to counter the spread of the infection have influenced the activity of the Department of Emergency Surgery of "San Giovanni di Dio and Ruggi d'Aragona"University Hospital in Salerno (Italy). Logistic regression and statistical analysis were used to compare the pre-pandemic data (year 2019) with that recorded at the height of the pandemic (year 2020). The results show that Diagnostic Related Group (DRG) weight and urgent hospitalization increased significantly in 2020. © 2021 ACM.
